Anyone can make an appointment at Whole Child LA. All appointments are prioritized on the basis of medical need.

New Patient Appointment

To make a new patient appointment with Whole Child LA, you can do one of the following:

2. If needed, you can call (310) 489-3119.
3. Fill out the online request form given below.  

 We promise to get back to you as soon as possible.  After confirmation of eligibility, please make sure to:  

1. Sign the medical release forms
2. Fill out the parent and patient questionnaires
