Anyone can make an appointment at Whole Child LA. All appointments are prioritized on the basis of medical need.

New Patient Appointment

To make a new patient appointment with Whole Child LA, you can do one of the following:

  3. Fill out the online request form given below.  

We promise to get back to you within 24 hours, if not sooner. After confirmation of eligibility, please make sure to  

  1. Sign the medical release forms
  2. Fill out the parent and patient questionnaires
